- Photo TX issued license OR
- US issued license
Optional at time of Application, Required by First Day of Class
Proof of U.S. Citizenship or Lawful Presence
- Birth Certificate
- U.S. Passport
- U.S. Citizen Identification card
Proof of Residency in Texas (Must have 2)
- Utility bill, phone bill, rental agreement, etc.
- Must have name & physical address
Proof of Identity
- Photo Texas DL or ID
- U.S. Passport or Passport card
- U.S Citizenship certificate
Proof of Social Security Number
Texas Vehicle Registration and Proof of Insurance
- Registration Receipt and Insurance ID Card
Medical Examination Certificate for CDL driver
- Must be a DOT medical certificate - NOT A REGULAR PHYSICAL
